DSPS guidance explains that Form SBD-8102 is completed before conditional approval issuance and becomes part of the approved construction documents after approval. The guidance includes project information, Special Inspector information/credentials, items requiring Special Inspection, reporting frequency (continuous or periodic), and acknowledgments/signatures by designated parties.
